Spring 2020 Refund Request Forms Due

The Spring 2020 refund documents are linked below. To request a refund of your registration fees paid, follow these steps:

1) Open the Spring 2020 Season Refund Options document to determine how you want your refund processed.

2) Complete the form below labeled Player Registration Fee Refund Request. Make sure you fill in all of the required information.

3) Email the completed form to: [email protected] by Monday, April 20th.

Refunds will be processed in the order received. It may take between 1-2 months for all refunds to be processed. Incomplete Refund Request forms will delay processing.
